Patient Privacy Statement

At Imperfect Medical, your trust matters to us, and so does your privacy. This statement explains, in plain language, how we handle the personal and health information you share with us as a patient.

The information we keep

When you become a patient, we collect the information we need to care for you well. This includes things like your contact details, your health history, the notes from your visits, and any results or records that are part of your care.

How we use it

We use your information to provide and coordinate your care, to communicate with you, and to run our practice responsibly. We limit who can see your information to the people who need it to help you, and we take reasonable steps to keep it secure.

When we share it

We do not sell your information, and we do not share it to advertise to you. We share your information only in a few situations: when you ask us to or give us permission, with other providers involved in your care at your direction, and when the law requires or allows it, for example to protect someone's safety. We follow the privacy laws that apply to our practice.

Your information belongs to you

You can ask to see or get a copy of your records, ask us to correct something that is wrong, and ask questions about how your information is handled. We will do our best to help.
